Family Case Management

what is family case management?

Family Case Management consist of Long term or Short term services, provided to migrant and seasonal farmworkers, or other low income rural households. Short Term Case Management consists of a plan that takes care of immediate crisis situations to stabilize households within a 30 day period. Services under this category are provided once every 12 months if qualified. Long Term Case Management is much more extensive and can continue 3 a 12 months in duration while the family is working on a number of situations/problems to stabilize the family and its economic position that will end in a positive upward mobility of the family and its economic viability.

The program consists of eligibility determination, assessing other individual/family needs and referral for additional services. Once eligibility is established and if funding is available, the case manager will work on service requests. Based on assessment, the Case Manager will provided 1 a 4 referrals to other agencies or other PPEP programs for other needs/services to begin the process of economic stability and the begin the road to self-sufficiency.
